How Do I Join?

·        Complete the Birthday Club Application Form

·        Submit your application online or e-mail to birthdayclub@mychoice2000.com

·        You will receive a confirmation e-mail with your Member ID within 48hrs

·        Make four qualifying purchases within any twelve month period

·        A qualifying purchase is any purchase made starting the day your application is submitted and all purchases made for the next twelve months with Birthday Club Member checked.

·        Qualifying purchases can be made by you or anyone using your Birthday Club Member ID #

·        All qualifying purchases whether gift basket or floral arrangement adds up

·        Be Sure to check Birthday Club Member and enter your ID# at the time of purchase

·        Your fifth purchase (equal to the average of your four qualifying purchase) is FREE
